The following sections are applicable to individuals age 18 and over and to children under age 18 where criteria are appropriate.

Major dysfunction of a joint(s) (due to any cause)
Reconstructive surgery or surgical arthrodesis of a major weight-bearing joint

Fracture of the femur, tibia, pelvis, or one or more of the tarsal bones
Fracture of an upper extremity
Soft tissue injury (e.g., burns)

2.00 Special Senses and Speech:

Visual disorders;

3.00 Respiratory System:

Chronic pulmonary disease;
ventilator y disease;

sleep-related breathing disorders;
lung transplant

4.00 Cardiovascular System:

chronic heart failure;
ischemic heart disease;
recurrent arrhythmias;
symptomatic congenital heart disease;
heart transplant;
aneurysm of aorta or major branches;
chronic venous insufficiency;
peripheral arterial disease

5.00 Digestive System:

Gastrointestinal hemorrhaging requiring blood transfusion;
chronic liver disease;
short bowel syndrome;
weight loss due to any digestive disorder;
liver transplant

chronic anemia;
sickle cell disease;
chronic thrombocytopenia (low platelet count);
hereditary telangiectasia (small dilated blood vessels);
coagulation defects such as hemophilia;

aplastic anemias with bone marrow or stem cell transplantation
